Participant Acknowledgment

1) ASSUMPTION OF RISK. I understand and acknowledge that participation in this event carries inherent risks. These risks include, but are not limited to serious injury, emotional distress, death from falls, strains and sprains, improper use or failure of equipment, failure to follow instructions, and negligence by myself, the presenter, LoveYourBrain Foundation, Inc., or others. I, the undersigned, understand that yoga, mindfulness, meditation, and other topics covered in this event are not substitutes for medical attention, examination, diagnosis, or treatment. I understand that I must judge my own capacities. If I experience any pain or discomfort, I will stop. I knowingly and freely assume all responsibility for any risks, injuries, costs, or damages, known or unknown, which I might incur as a result of participating in the event. I, for myself, my heirs, assigns, and legal representatives, agree to release, hold harmless, and indemnify (including costs and attorney’s fees), to the fullest extent permitted by law, LoveYourBrain Foundation, Inc., its directors, officers, employees, instructors, independent contractors, agents, and representatives, and their successors and assigns, against any claim—including negligence on the part of those being released—related to my participation in the event. 2) PHOTOS & VIDEOS. I acknowledge that during my participation, my voice may be recorded or I may be filmed (collectively called "My Recording") by LoveYourBrain. I consent to LYB posting this content in print or online and sharing My Recordings with various people within the LYB organization and with other people who are not part of the LYB organization for the purpose of learning and any other legitimate reasons. 3) VIRTUAL EVENTS. I acknowledge that I am participating in the activities outlined by this virtual event by my own free will and at my own personal risk. I will not participate in a virtual event unless I am medically able, and by my signature, I certify that I am medically able to participate in this event. 4) NON REFUNDABLE. I understand that LoveYourBrain is a 501(c)3 nonprofit organization, and that all money paid to or raised for LoveYourBrain supports its mission. I understand that my registration fee and all money fundraised is non-refundable, and by signing this waiver, I consent that I am not entitled to a refund if the event is canceled before or during the event. 5) LEGAL ACTION. I agree that if a lawsuit is filed arising from or related to my participation in this event, the lawsuit must be filed in Vermont. The lawsuit will be governed by and resolved according to the laws of Vermont. In consideration of being allowed to participate in the event, by checking this box I AGREE to the above terms. 6) FUTURE COMMUNICATIONS. I acknowledge that I am providing my email during registration for this event, and that I may receive additional communications from LoveYourBrain including being thanked for my support, updated on my impact, and made aware of other opportunities. I understand I can opt out of these emails at any time.
